Cancer Survivorship Care

Parkland’s Cancer Program has launched several clinics for patients who have completed their cancer treatment. In these clinics the patient will receive an assessment to the providers provide support for the patients’ needs for their next phase of their care.

Before these clinics, patients will receive questionnaires to help with the assessment through their MyChart account. It is important to fill these out to help the providers prepare for the visit.

During these clinics, patients may be seen by an advanced practice provider, social worker, registered dietitian nutritionist, physical therapist, and nurse navigator. This team helps provide the patients information about resources available and possible future side effects to watch for.

Patients will also receive their survivorship care plan at this visit which will help guide the patient through their survivorship care.

What do the survivorship care plans do?
These care plans provide clear information for the patient about their cancer diagnosis, the treatment they received, and what the next steps will be.
